About this role
The Mental Health Social Worker will provide client-driven health and social case management assistance to children and youth in the community and those clients attending the Health Centre to achieve safe and reasonable goals that will optimize clients' health and wellness and improve their quality of life. The Mental Health Social Worker works in collaboration with clients, their families and community and government agencies, and works closely with the Mental and Addictions teams.
