About this role
The Case Manager provides social case management support as a primary care provider to the client. The population for this project are those who have difficulty engaging or have disengaged from care, involved in substance abuse/who are impacted by substance use disorder and Mental Health issues. Utilizing an outreach approach, the case manager provides client services including psychosocial assessments on intake, addiction assessments, client goal setting/skill building, and coordination/support of health/social services.
