Reporting to the Primary Health Care (PHC) Regional Director (or Designate such as Community Health Services Manager), PHC Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide visiting PHC services to sites within the service area.
