Reporting to the Director, Primary Health Care Teams and in collaboration with physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens) as determined by community and program needs.
