The RN Case Manager will report to the Primary Health Nursing Coordinator and Manager of Primary Health Care. This position will be an integral part of the Primary Care Health Team within the Pinehouse Health Centre. Although the primary base of work will be in a primary care setting, some community outreach and programming will be required. The RN Case Manager will also provide support in areas of public health and communicable disease management. The focus of the RN Case Manager will be to offer patient case management through holistic care coordination within a multidisciplinary health care
