About this role
The RN Case Manager ( RNCM) will report to the Primary Health Nursing Coordinator and Manager of Primary Health Care. Although the primary base of work will be in a primary care setting, some community outreach and programming will be required. The RNCM will also provide support in areas of public health and communicable disease management. The focus of the RNCM will be to offer patient case management through holistic care coordination within a multidisciplinary health care team.
