Reporting to the Program Manager of the Acquired Brain Injury department within the Rehabilitation and Supportive Care Services portfolio and as a member of a multi-disciplinary team, the Healthcare Social Worker provides a diverse range of interventions to clients and their families enabling them to achieve their optimal level of functioning and health. These interventions include psychosocial assessments, adjustment therapy, supportive therapy, crisis counseling, resource counseling, discharge planning and case management. The Health Care Social
