Reporting to the Manager, the Assessor Coordinator - Degree is responsible for the coordination and evaluation of LTC screenings, and client supportive case management. The Assessor Coordinator degree will ensure that the case management services are consistent with the policies, vision, mission and philosophy of the SHA.
